Effect of weight-loss on the dynamic hyperinflation in obese asthmatics

Abstract Background :Evidences suggest the presence of obesity in asthmatic patients seems to impair the respiratory mechanic, mainly during exercise. In addition, it seems that the presence of dynamic hyperinflation (DH) is increased in obese asthmatics; however, the effect of weight-loss in the DH in these individual remains unknown. Aim :to assess the effect of weight-loss on change of the DH in obese asthmatics. Methods : 45 patients withmoderate or severe asthma with obesity grade II (BMI³ 35kg/m2 ) under optimized medication and clinically stable were submitted to a weight-loss lifestyle program composed by nutritional, psychological and exercise training, twice a week for 3 months. Before and after the interventions, patients performed a cardiopulmonary exercise test to measure peak VO₂and an anthropometric evaluation (day 1) and a submaximal exercise test (75% of maximum load) until exhaustion and the presence of DHwas evaluated every 2 minutes measuring inspiratory capacity (IC) (day 2). Results :Before intervention, 34 patients (75.5%) presented DH observed by a reduction in the IC. After intervention, 19 patients (42%) presented a weight lost more than 5% of weight loss. From those that presented weight loss 14 patients (73.6%) presented a decrease in the DH after intervention, changing the IC from -17.42%±13.81 to -9.16±14.52. Conclusion : Our results suggest that is necessary to lose more than 5% of the body weight to improve the IC and consequently the DH in obese asthmatics.
