Abstract

Background. Effect of inhaled glucocorticosteroids (IGCC) on the course of the disease and quality of life in patients with bronchiectasis in remission, has not yet been studied.Aim. Investigate the efficacy of combined inhaled drugs (IGCC + long-acting beta2-agonist) on quality of life, microbiological and functional parameters in patients with mixed bronchiectasis in remission.Methods. A 6-month study in 45 patients was researched. Mixed bronchiectasis verified by high resolution CT scan of the chest. Depending on the dose of inhaled corticosteroids patients were randomized: fluticasone propionate 250 mcg twice daily and 500 mcg twice daily. Quality of life was assessed by St. George's Respiratory Questionnaire. In the group treated with fluticasone propionate 1,000 mcg per day, after one month significantly decreased dyspnea, sputum production, cough frequency and the need for the use of short-acting beta2-agonists, but not observed changes in lung function: forced expiratory volume in 1 second, Gensler index, and microbiological profile of sputum (Staphylococcus aureus, Pseudomonas aureginosa).Results. In this group of patients was significantly improved quality of life (p <0.001) within three months of treatment. Community-acquired pneumonia is not registered.Conclusions. Application twice a day for 500 mcg fluticasone propionate in combination with long-acting beta2-agonist controls symptoms after the first month of treatment in patients with mixed bronchiectasis in remission. This result is confirmed by significant improvement of life quality.
