Abstract

Objective : Setting up a mobile outreach diagnostic unit to facilitate diagnosis of TB in thesocioeconomically deprived population of rural IndiaMaterial & Methods : A mobile diagnostic unit with x-ray facilities was developed and went out on aweekly rotational basis from Medanta Hospital, Gurgaon to 6 primary health care centres (PHC) inDistrict Rewari in the state of Haryana. The District TB Officer and the Medical Officers in therespective PHC identified patients suspected to have pulmonary tuberculosis but with negativesputum. These patients were jointly reviewed in a clinic by the TB Officer and the external doctor (fromMedanta Hospital). All the patients which clinical suspicion of pulmonary TB and chest x-raysupporting the diagnosis, were taken into the DOTS program. The program ran from May 2014-November 2014.Results : In this 7 month period, a total of 335 patients underwent chest X-ray.201 Patients (60%)were male and 134(40%)were females.84(25%) had history of prior tuberculosis.109 (33%) patients had chest X-ray suggestive of pulmonary TB, corroborated by clinical features and were taken intothe DOTS program. The existing diagnostic rate in sputum negative patients in Rewari district is36/100,000 (national target - 95/100,000) and the use of a mobile X-ray unit increased this to56/100,000.Conclusion : Setting up the mobile diagnostic unit 1) significantly increased the diagnosis ofsputum negative TB in District Rewari 2) facilitated and enhanced the government led DOTSprogram. This program can be replicated on a larger scale to increase diagnosis of TB which is ascourge in India.
