Abstract The MOSAIC is a multinational, double-blind study comparing the effectiveness of a 5-day oral moxifloxacin (MXF) course to that of a 7-day standard oral antibiotic regimen (amoxicillin, clarithromycin or cefuroxime-axetil), in the treatment of AECB. CB patients with history of smoking and recurrent AECB episodes, were enrolled into the study and their infection-free baseline prospectively established. After stratification with respect to corticosteroids administration, patients were randomised into two parallel groups (MXF or comparator) if they suffered a type 1 Anthonisen AECB episode. Clinical response was assessed 7-10 days post-treatment. The time to regain the pre-exacerbation status was also evaluated as well as the time until next AECB, throughout a 9-month follow-up period. 1935 patients were enrolled. After a 12-month monitoring period, 733 patients had an exacerbation and were randomised. Duration of CB and number of previous AECB episodes were found to be predictive factors for the occurrence of type 1 AECB episode in this CB population. Characteristics patients are summarised below:
Recruited Randomised p-value Age (years) 63.4 63.2 0.64 Sex (%males) 68.2 67.9 0.84 Duration of CB at enrolment (years) 11.7 12.5 0.04 AECB episodes in last year 2.9 3.1 0.0008 FEV1 (% pred) 53.7 54.2 0.58
