Bacterial infections in patients with acute exacerbation of chronic obstructive pulmonary diseases

Abstract The aim of this study was to examinated the prevalence of bacterial infections in patients (pts) with acute exacerbation of chronic obstructive pulmonary diseases (COPD). During 2000 year in Institute for Pulmonary Diseases and TB there were 5415 admissions and 765 (14,1%) pts for COPD. For investigation we selected 146 pts with acute exacerbation of COPD and clinical findings for respiratory infections (48-32,8% females, 98-67,2% males, mean age 60,3 year). We analysed presence of specific micro-organisms in sputum cultures. In 54 (36,9%) pts positive sputum cultures indicated bacterial infections. Klebsiella (21-38,9% pts), Haemophilus influenzae (14-25,9% pts), E.coli (2-3,6% pts) Pseudomonas aeruginosa (12-22,3% pts) were most frequently isolated pathogens and more than one microbial agent was found in 5 (9,3%) pts (Streptococus pneumoniae, Acinetobacter, Enterobacter ). All pts were selected in various stages severity of COPD, using newly established guidelines from the Global Initiative for Obstructive Lung Disease (GOLD). Among 54 pts with bacterial infections in acute exacerbation of COPD there were 26(48,1%) pts in stage III, 15(27,8%) pts in stage II B, 8(14,8%) pts in stage II A and 5(9,3%) pts in stage I. We concluded that among pts with acute exacerbation of COPD and clinical characteristics of respiratory infections there were 36,9% pts who had presence of specific micro-organisms in sputum cultures. Pts were most frequently in III and II B stage severity of COPD.
