Abstract Previous studies performed in Spain and other countries showed that the real incidence of occupational respiratory diseases (ORD) is considerably underreported. This incidence is mainly obtained from the compulsory and official system of reporting occupational respiratory diseases. Our main aims were:- To create a fully implemented voluntary registry of the ORD in Castilla –LaMancha (CLM) during the period from the 1st of January to the 31st of December 2010 that allowed us:- To get to know the incidence of ORD in CLM during 2010.- To compare the incidences of ORD in CLM obtained from the different notification registry systems: voluntary versus compulsory official registry. It was elaborated a Notification File that should be complete every two months by every member physicians of the Pneumology, Allergy and Occupational Medicine that voluntarily accepted to participate in the study, collecting information from the new cases (incidence). This study involves the whole practice of the public and private medicine in CLM (Albacete, Ciudad Real, Cuenca, Guadalajara and Toledo). The register is population based. The incidence is calculated over the active population (from the age of 16 to 64) excluding housewives. The active population in CLM was 990.400 workers in 2010. There were reported 134 new cases. The incidence of ORD in CLM was: 135,30 and 29,28 cases/million workers and year in voluntary registry versus compulsory and official system, respectively. It has been created the voluntary registry of the ORD in CLM in 2010 and it appears to be as an useful tool, even more than the official registry system.Having reviewed the literature, we have not found new data after the results of this study.
