Abstract Introduction: Pleural plaques (PPs) and diffuse pleural thickening (DPT) are common manifestations of asbestos exposure. The impact of PPs and DPT on quality of life (QoL) and respiratory symptoms has not been well defined. Aims: To evaluate respiratory symptoms, QoL and lung function in subjects with DPT and PPs compared with healthy controls. Methods: Individuals with PPs or DPT (classified according to the ATS statement on non-malignant asbestos-related disorders) and controls completed full lung function, the St George's Respiratory Questionnaire (SGRQ), SF-36 Health Survey and 6-minute walk test (MWT). Results: 73 subjects were recruited (24 PPs; 21 DPT; 28 controls). Results are shown in Table 1.Table 1
Controls n=28 PPs n=24 DPT n=21 Age (years) 59 (46) 69 (40)# 70 (26)* FEV1(%) (Mean(SD) 103.5 (12.6) 96.1 (11.5)# 80.1 (18.4)*^ FVC(%) (Mean(SD) 109.7 (12.5) 100.7 (10.4)# 87.9 (18.8)*^ FEV1 /FVC (%)(Mean(SD) 74.9 (7.6) 72.9 (6.4) 71.0 (8.3) SGRQ Symptom score 9.9 (86.0) 18.4 (76.8) 25.3 (81.0)* SGRQ Total score 3.6 (66.4) 6.9 (58.0) 19.0 (84.4)* SF-36 Mental Component Score (MCS) 55.2 (36.4) 57.0 (29.9) 51.5 (36.3) SF-36 Physical Component Score (PCS) 55.2 (30.7) 48.7 (34.9)# 44.6 (38.4)* 6 MWT distance (%predicted) (Mean(SD) 100.4 (13.3) 92.9 (45.1) 90.2 (21.2)
In the DPT group, SGRQ symptoms score correlated inversely with FEV1(%)(r=-0.66, p<0.01) and FVC(%)(r=-0.53, p<0.05). SF-36 PCS correlated positively with FEV1(%) (r=0.72, p<0.01) and FVC(%) (r=0.69 p<0.01). Conclusions: Subjects with DPT and PPs have worse QoL compared with healthy individuals. However, the impact of PPs on QoL was smaller than DPT. Median(range), #p<0.05 PPs v controls, *p<0.05 DPT v controls, ^p<0.05 DPT v PPs
