Neutrophilic inflammation in severe asthmatics and COPD patients with the same severity of airway obstruction

Abstract

Asthma and COPD are two different obstructive lung diseases caused by different types inflammation. However, recent reports have similarities of inflammation characterized the two entities. The aim of this study was to investigate the airway inflammation of severe asthma and to compare with severe COPD. Methods. We studied a group of 13 severe asthmatic patients, aged 56±]10 years, FEV1 49±]13 (% of pred), and another group of 15 COPD patients (65±]26 pack/years), non atopics, aged 69±]8 years, FEV1 45±]12 (% of pred), with irreversible airway obstruction. Both groups had similar daily intake of inhaled corticosteroids. A differential cell count obtained by induced sputum and May-Giemsa-Grunwald stained slides was performed and IL-8 and GM-CSF levels were measured using enzymatic immunoassay method (trousse de dosage immunoenzymatique enzyme immunoassay kit for IL-8 and GM-CSF from Immunotech) Results. Statistical significant difference between severe asthmatics and COPD patients was found in the percentage of neutrophils, p<0.03, median (range) (%) 51 (35-98.9) vs. 72 (51-82), but neither in the concentration of IL-8, p<0.379, median (range) (pg/ml) 3229 (569-32810) vs. 6522 (610-57295), nor in the concentration of GM-CSF, p<0.626, median (range) (pg/ml) 42 (0-66) vs. 62 (0-919). Conclusions: Our findings showed that though severe asthmatics have less sputum neutrophilia compared to COPD patients with the same level of airway obstruction, the levels of IL-8 and GM-CSF were not significantly different.
