Extracellular and intracellular magnesium before and after methacholine-induced bronchoconstriction in asthmatics

Abstract Magnesium (Mg) is important in the regulation of bronchomotor tone, mast cell secretion and neuromuscular conductivity. Mg deficiency is associated with airway hyperreactivity and wheezing and impairment of lung function. The aim of our study was to investigate the levels of extracellular (plasma) and intracellular (erythrocytes) Mg and its possible relationship with airway hyperreactivity in patients with asthma. Methacholine challenge was performed by the dosimeter method and provocative dose causing a 20% fall in FEV1 was measured. The level of Mg in plasma and erythrocytes in 23 patients with asthma and in 17 normal subjects was studied by a calmagite colourimetric assay, both at baseline and when FEV1 had fallen by 20 %. In asthmatic patients, Mg levels were significantly lower in erythrocytes before and after methacholine challenge (p<0.0001), whereas plasma levels did not differ (p=0.43). In erythrocytes,the levels of Mg were not correlated with the degree of bronchial hyperreactivity. In conclusion, low erythrocytes Mg level may be associated with airway hyperreactivity in patients with asthma.
