Abstract

BACKGROUND: Pulmonary embolismis probably the most common acute cardiovascular disease after cardiac ischemic syndromes and stroke, with increasing frequency during the past years. It is an important cause of morbidity and mortality particularly in the hospital setting. However, its diagnosis has often eluded the physician primarily because of failure to suspect the event. This study describes the clinical profile of patients diagnosed with pulmonary embolism in St. Luke's Medical Center, a tertiary hospital in a developing country like the Philippines. METHODOLOGY: Records of patients diagnosed with pulmonary embolism based on the Disease Index Report of the Medical Records Department of St. Luke's Medical Center from September 2000 to September 2001 were reviewed. The following pertinent data were recorded and analyzed: demographic features, predisposing factors, signs and symptoms, diagnostic and therapeutic management. RESULTS: A total of 15 cases were recorded with prolonged immobilization as the most common predisposing factor. The most common symptom and physical sign is dyspnea and tachypnea, respectively. Chest X-ray, electrocardiography, blood gas, and echocardiography results were varied. Venous dupplex scan of the lower extremities were positive in 89%. 36% of those subjected to ventilation/perfusion scan showed high probability scan. 100% of those subjected to pulmonary angiogram revealed bilateral pulmonary embolism. CONCLUSION: The diagnosis of pulmonary embolism is still based primarily on clinical grounds. A high index of suspicion is of outmost importance. Further studies are recommended for better understanding of the disease process.
