Abstract The abundance of HROQL questionnaires makes a choice of the best tool difficult. Concurrent validation of new language versions of two HRQOL questionnaires may allow for rapid comparative assessment of psychometric properties of both questionnaires. We conducted a comparative validation of Polish versions of the two HRQOL questionnaires in patients with obstructive sleep apnea syndrome (OSAS) - the Calgary Sleep Apnea Quality of Life Index (SAQLI) and the Functional Outcomes of Sleep Questionnaire (FOSQ). Following translation and back-translation of both questionnaires, 72 patients with OSAS underwent sleep studies, HRQOL measurements using SAQLI, FOSQ, SF-36, and Global Health Rating Scale (GHRS), and assessments of their sleepiness using Epworth Daytime Sleepiness Scale (EDSS) and Rotterdam Daytime Sleepiness Scale (RDSS). Both FOSQ and SAQLI showed good reliability, with Cronbach α coefficients of 0.87-0.93 for FOSQ and 0.84-0.91 for SAQLI. Both FOSQ and SAQLI scores significantly correlated with EDSS, RDSS, SF-36 and GHRS scores, which confirmed validity of both questionnaires. Both FOSQ and SAQLI showed good item-internal consistency; SAQLI was superior to FOSQ in item-discriminant validity. All HRQOL and physiological measurements were repeated after 4 weeks. Polish version of SAQLI showed similar stability, but superior responsiveness to changes in health status over FOSQ. Our study showed validity of Polish versions of FOSQ and SAQLI. The simplicity of FOSQ makes it an adequate tool for screening and cross-sectional assessments of HRQOL. Excellent measurement properties make SAQLI preferable for longitudinal studies.
