Abstract

Home-base health care has grown over last few years and patients with acute exacerbation of COPD can be safely managed at home. Intermediate Respiratory Care Team has been created in West Kent 1 year ago. Over the first 6 months 30 patients age 74 ±] 1.7 years with exacerbation of COPD have been referred to the team by General Practitioners. They underwent assessment at home and subsequently the treatment was modified by respiratory team. 3 patients were referred back to GP since they were suffering from different condition. The mean follow-up of patients was 16 ±] 2.7 days.
Only 30% of patients were able to performed spirometry due to severity of the illness and therefore this measurement was not very usefull in assessement of effectiveness of the treatment.More usefull measurements were: respiratory rate, saturation and heart rate. Thus respiratory rate change from 25 ±] 1.1/ min at assessment to 20 ±] 0.6/ min on discharge ( p < 0.001). Also there was significant increase in Sa02 from 91 ±] 1.1 % at assessment to 94 ±] 0.6 % on discharge ( p = 0.0065). Heart rate fell from 92 ±] 2.8 beats/min at assessment to 84 ±] 3.1 beats/min at discharge ( p < 0.0001).
3 out of 30 patients did not improve and they were referred to hospital. All patients were satisfied with domicilary care. Home treatment of exacerbation of COPD can provide alternative to hospital treatment.
