Induction-related changes in sputum neutrophils are associated with increased sputum levels of IL-8, but not sICAM

Abstract

Induced sputum (IS) is a valuable tool to study inflammation of the lower respiratory tract. However, the procedure of sputum induction itself has repeatedly been shown to induce changes in the cellular composition, in particular neutrophils, of IS. The mechanisms associated with these changes are not fully understood. IL-8 is a pivotal cytokine for recruitment and chemotaxis of neutrophils, and homing of neutrophils requires adhesion promoting factors like ICAM. 10 non-smoking, non-atopic, non-asthmatic subjects who had previously demonstrated increased sputum neutrophilia following sputum induction inhaled 4% hypertonic saline for 30 minutes on two consecutive days. Sputum cell differentials were count, and supernatant levels of IL-8 and sICAM quantified by ELISA. Compared with baseline values, there was a significant rise in sputum neutrophils after 24 hours (39±]6 vs. 18±]2 %, p=0.005), whereas other cells remained unchanged. Moreover, sputum levels of IL-8 were increased (14.2±]3 vs. 9.4±]2.4 ng/ml (baseline), p=0.06). Sputum levels of sICAM were not significantly different on day 2 (8±]2 vs 7.7±]2 ng/ml, p>0.5). In conclusion, the induction-related rise in sputum neutrophils is associated with increased sputum IL-8, but not sICAM. Whether this observation represents a causal, or merely associated phenomenon, remains to be elucidated.
