Bronchial challenge tests in asthmatics sensitized to cat: importance of large particles in immediate bronchial response

Abstract Our aim was to compare bronchial responses according to the particle size of droplets carrying major cat allergen (Fel d 1) in intermittent asthmatics sensitized to cat. 3 nebulizers were selected with MMAD of 1.4, 4.8 and 10.3 μm. Allergen concentration was calculated to obtain the same amount of Fel d 1 per inhaled dose. A dosimeter Spira Elektro II nebulizer was used. For each patient, cat BCTs were performed blindly on 3 occasions with 3 nebulizers randomly selected. A metacholine BCT was performed 24 hours before cat BCT. 19 patients were selected, 11 women and 8 men, mean age 27 years, comparable to themselves in terms of NSBHR, FEV1, FEF25-75, PEF and dyspnea Borg scale before the 3 cat BCTs. PD20 cat allergen was 441 ng of Fel d 1 with the 1.4 μm nebulizer, 82 ng with the 4.8 μm nebulizer and 22.5 ng with the 10.3 μm nebulizer (p=0.001). On the other hand, inhalation of small particles caused a more severe fall in FEF25-75 24 hours after cat BCT than large particles (p=0.006). In conclusion, immediate bronchial response seems to be localized in large airways. It seems more appropriate to perform BCT with large particles carrying allergens.
