Airway remodelling in asthma is related to particular characteristics of airway inflammation

Abstract Introduction. Reticular basement membrane (Rbm) thickening in asthmatic ebb is a commonly reported feature of airway remodelling, but its relationship with inflammation is ill defined. We have recently shown that Rbm thickening was related to BAL fluid mast cell numbers and that while inflammation was down regulated by 3 months inhaled fluticasone propionate (FP 750 μg bd), an effect on Rbm was only noted at12 months (1. Ward et al Thorax March 2002 In Press ). Hypothesis . Ebb provides complimentary information to BAL, that support a link between inflammation and airway remodelling. Methods A systematic, cross sectional analysis in 35 symptomatic, mild to moderate atopic asthmatics. BAL and ebb quantified for inflammatory cell profiles and ebb for Rbm thickening (1). Matrix analysis of individual univariate correlations, with subsequent "best subsets" multiple regression. Results. BAL and ebb inflammatory terms explained 43% of the variability in Rbm thickening ; Log%BAL mast cells,25%; ebb (AA1) mast cells,10%; log EG2 positive ebb eosinophils, 8%. This was better than our previous model, restricted at that time to BAL inflammation, which explained a total 23% of the variation in Rbm. Discussion : Ebb and BAL provide different and complimentary information linking airways inflammation to Rbm thickening in asthma. The emergence of BAL parameters, relating to ebb Rbm thickening, indicate that inflammatory processes in both the airway wall and on the epithelial surface may be relevant.
