Abstract

The are many factors which can influence on survival of lung cancer patients. In huge population based data gender , age, performance status, clinical stage of the disease, histology and treatment were independent predictor of survival.
The goal of this study was to assess the significance of smoking in survival analysis of unselected population of lung cancer patients.
Patients and methods: The Pulmonary Outpatients Departments supplied data on 20561 lung cancer patients diagnosed in Poland in 1995 to1998. Data regarding demographic, smoking, histology, performance, clinical stage of the disease, treatment and survival were obtained.
Results: Information on smoking history was available on 15527 cases. There were 12514 smokers and 552 non-smokers . Survival analysis of one year(45.6% vs. 37.6% ; p<0.001) and two years ( 15.9% vs. 12.3% ; p<0.001) survival revealed that non-smokers had a significantly better survival than smokers.
In multivariate analysis smoking (Relative Risk – RR=1.2), male gender (RR=1.21), age over 50 years( RR=1.19), nonsurgical treatment (RR=3.03), small cell histological type of lung cancer(RR= 1.36), performance status 2 (ECOG scale)(RR=1.63) , 3+4( RR=2.73) , and clinical stage of the disease 2(RR=1.45), 3(RR=1.86), 4(RR=2.73) were negative significant independent prognostic factors in lung cancer patients.
Conclusion : Smoking is a negative predictor of survival in lung cancer patients.
