Effects of muscular function and tolerance to exercise in severe stable COPD using bilevel noninvasive mechanical ventilation: a pilot study

Abstract In the last decades, with a best comprehension of the respiratory mechanic alterations and its consequent influence to ventilatory muscle function and exercise tolerance. Many authors have demonstrated that the use of Bi-level NIV decreases work of breath, improves gas exchange, reestablishes respiratory muscles function with a better performance for physical activities leading to a better quality of life for the patients.The purpose of this study was to investigate the effects of the intermitent use of Bi-level NIV in severe stable COPD patients considering muscle function and exercise tolerance. Eight male severe and stable COPD patients (FEV1=32.3%) were submitted to ventilatory support (BiPAP®) for a period of 3 hours during four days in a row. We evaluated Spo2, HR, MV, VT, inspiratory muscle strength and effort perception index through the use of pulse oximeter, respirometry, vacuometry, six minutes walk test and Borg scale respectively. During monitorization of Bi-level NIV we observed a decrease in breath rate and increase in Spo2 ( p =0.000 and p = 0.000). However we didn't find a significant result considering HR ( p = 0.664), respirometry with MV ( p = 0.327), VT ( p = 0.484), BR ( p = 0.932), Pimáx ( p = 0.131), WD ( p = 0.285), TT ( p = 0.398) and EPI ( p = 0.145). This study intends to be a pilot program deserning a greater attention and methodological precision to standardize a treatment protocol. SUPORTED: CNPq / UFPE , *PIBIC/ CNPq/ UFPE and AGA Healthcare
