Psychosocial determinants of effectiveness of rehabilitation in COPD patients

Abstract Rehabilitation has been shown effective in improving symptoms, exercise performance and quality of life in COPD. Some patients seems to respond better to rehabilitation than others. We hypothesized that anxiety and depression (HAD-questionnaire), social support (Social Support List, SSL), and adjustment problems (Psychosocial Adjustment to Illness Scale, PAIS), could contribute to the individual responsiveness. 47 COPD patients (FEV1 40±]16%pred) referred with complaints of dyspnea, were included in a multidisciplinary pulmonary rehabilitation program. Pulmonary function, exercise capacity (Wmax, 6MWD), and quality of life (CRDQ) were assessed before and after 3 months rehabilitation. Baseline Anxiety and Depression were respectively 8.2±]4.4 and 8.6±]4.4. PAIS was 58±]17 and SSL, 69±]15 points. Traveling distance was 27±]22km, 14% of the patients lived alone, and 25% of the patients were current smokers. Rehabilitation improved exercise performance (Wmax 4±]12Watts, p=0.03, 6MWD 32±]73m p<0.01), CRDQ (11±]12points, p<0.001) and HAD anxiety (-1.4±]3.0, p<0.01) and depression (-2.6±]3.3, p<0.01). In single regression analysis non of the psychosocial variables was related to the outcome of rehabilitation. Traveling distance nor smoking status were related to the outcome. It is concluded that baseline psychosocial factors do not predict the outcome of rehabilitation. The multidisciplinairy approach in our program might allow patients with less favorable psychosocial conditions also to benefit from rehabilitation.
