Determinants of activities of daily living (ADL) in COPD patients

Abstract Patients with COPD experience a reduced capacity to perform ADL. Symptoms and reduced physiological capacity might contribute to this,but, determinants of ADL were never studied. Continuous unsupervised monitoring of ADL is a promising new method for assessment of ADL. A triaxial accelerometer ADL monitor (Dynaport McRoberts, The Hague, The Netherlands) registers 4 main ADL categories: sitting (SS), standing (ST), locomotion (walking WA), and lying (L) during 24 hours. We assessed 24-hour ADL with the Dynaport in 26 patients with COPD (FEV1 41±]17%pred) with complaints of dyspnea. Maximal exercise performance (Wmax 40±]17%pred), 6-minute walking distance (6MWD 53±]23%pred), quadriceps force (QF, 64±]27%pred), quality of life (CRDQ, 72±]18points) and Percieved Functional Status and Dyspnea Questionaire (PFSDQ-M, 5.3±]2.1) were assessed. Evaluation of ADL with the Dynaport revealed time for SS (481±]218min), ST (182±]110min), WA (41±]26min) and L (575±]217min). In single regression, significant relationships were observed between ST and WA on the one hand and FEV1, Wmax, 6MWD, and QF (0.38<R>0.74). No such relationship was observed for SS, and L. PFSDQ-M was inversely related to ST (-0.70, p<0.05) and WA (-0.52, p<0.05) and positively to SS (0.49, p=0.06). These findings suggest that active components of ADL (standing and walking) are related to physiological variables in patients with severe COPD. Improving these variables may therefore result in improved ADL.
