Abstract
48 patients with NSCLC and ipsilateral pleural effusion underwent thoracotomy between 1990 and 1999. Hystological types were squamous cell carcinoma in 23 cases (0,48) and adenocarcinoma in 25 cases (0,52). In 35 patients (0,71) effusion was found by roentgenogram and CT of the chest, without evidence of carcinomatosis by pleural fluid cytology. Effusion in remaining 13 cases (0,29) was found at thoracotomy.
In time of thoracotomy pleural metastases were diagnosed in 19 patients (0,40), there were surgically unresectable. From other 29 patients with paramalignant effusion 9 (0,31) were unresectable owing to mediastinal vessels involvement. 20 patients only undergone complete resection of tumor: 16 in stage IIIa (0,8) and 4 in stage IIIb (0,2). 15 pneumonectomies (0,75) and 5 lobectomies (0,25) were done. 2- and 5-year survival was 0,39 and 0,17.
The presence of a pleural effusion in lung cancer patients associated with poor prognosis. The number patients with paramalignant pleural effusion can be treated surgically.
