Sentinel node mapping in lung cancer patients using radioisotope technique

Abstract I presented this data at the ERS annual congress last year. Using the dye-guided method postoperatively, identification rate of the sentinel node (SN) was 93.4%, the accuracy was 94.7%,and it was considered that lung cancer surgery with a reduces range of the lymph node dissection was facilitated by determining the presence or absence of metastasis in the identified SN. Based on this data we are now examining the identification of SN using the radioisotope technique intraoperatively. Though the series is still limited to 11cases, we report the method and results to date. Under CT-guidance, Tc-99 tin colloid was preoperatively injected around the tumor circumference . The activity of Tc -99 was measured intraoperatively using gamma counter, and the lymph node in which the activity was the highest was defined as SN. If lymph node metastasis was detected, it was then determined whether the defined SN was the metastatic lymph node. The SN identification rate was 45.5%, and the accuracy was 100%. Since the identification rate was low, timing of Tc-99 injection was changed from just before surgery to the previous evening. SN were successfully identified in the resent 2 cases. Other merits of this changed is that the radioisotope that flowed into the trachea disappeared and the detection of background radiation decreased. In the future, further cases will be accumulated and a method of reducing lymph node dissection will be established.
