Induced sputum neutrophilia and increased neurotrophin levels in patients with cryptogenic fibrosing alveolitis

Abstract

Patients with cryptogenic fibrosing alveolitis (CFA) often have a dry cough, although the pathogenetic mechanisms which underly this symptom are unknown. We have previously demonstrated an increased cough reflex sensitivity to inhaled capsaicin and substance P in CFA, suggesting functional upregulation of respiratory tract sensory nerves (Thorax 2001;56:S134). In this context, we postulated that enhanced levels of neurotrophins in the respiratory epithelium might contribute to this upregulation. Methods. Induced sputa were obtained from 10 CFA and 9 control subjects. Selected sputum plugs were divided in two; one part was processed with 0.1% dithiothreitol to obtain cell counts. The remainder was combined with protease inhibitors and stored at -70[degree]C. Brain-derived neurotrophic factor (BDNF) was measured by ELISA (R&D Systems, Abingdon, UK). Cell counts are expressed as cells x106 /g sputum±]SD. Mean concentrations of BDNF are expressed as pg.ml-1±]SD. Non-parametric data were compared by Mann-Whitney U test. Results. The total cell counts were greater in subjects with CFA (5.9±]5.3)compared with controls (1.7±]1.4; p<0.01) as were absolute neutrophil counts (3.6±]4.4 vs. 0.36±]0.31; p<0.001) and lymphocyte counts (0.09±]0.08 vs. 0.02±] 0.02; p<0.02). Sputum BDNF levels were higher in subjects with CFA compared with controls (37.3±]12.1 vs. 23.7±]11.6; p<0.01). Conclusions. Sputum neutrophilia in patients with CFA suggests inflammation within the respiratory epithelium. Increased levels of BDNF suggest the inflammatory process provides an environment which could promote neuronal proliferation.
