The diagnostic value of the total adenosine deaminase(ADAp) and ADA1/ADAp ratio in the diagnosis of tuberculous pleural effusions

Abstract In the diagnosis of tuberculous pleural effusions it is possible to have a high diagnostic value with the histologic and bacteriologic examination of the pleura biopsy. In our study we have examined the contribution of ADA1/ADAp ratio on the pleural effusion total ADA (ADAp) enzyme value in cases of a diagnosis difficulty. In our prospective study we have taken 49 cases and 32 of them had a diagnosis of tuberculous pleural effusion (TPE). We have found the average ADAp value as 110,3 +/- 27,4; and the average ADA1/ADAp ratio as 0,16 +/- 0,08 in the TPE group; in the Non-TPE group the average ADAp value as 44,8 +/- 59,8 and the average ADA1/ADAp ratio as 0,42 +/- 0,25. We have taken >= 40 IU as the cut-off value for ADAp; and the [lte] 0,42 value for ADA1/ADAp. According to these values we have reached a 100% specificity and sensitivity when we have used (in cases of ADAp >= 40 IU and ADA1/ADAp [lte] 0,42) both parameters together. Only for ADAp, in the diagnosis of TPE, the specificity has been found as 82,3% and the sensitivity as 100%. As a result, depending on these findings, in the diagnosis of TPE, it can be thought that in cases of diagnosis difficulty the use of the ADA1/ADAp and ADAp parameters together according to only ADAp has a higher specificity value.
