The role of transbronchial lung biopsy in the early detection of tuberculosis

Abstract To assess the value of transbronchial lung biopsy (TBLB) in the early diagnostics of pulmonary tuberculosis (TB) we retrospectively analyzed 69 TB patients who had undergone TBLB during a six month period in 2000. Materials and methods: These were 15 women and 54 men aged from 19 to 76 years, in whom infiltrative (n=39), disseminated (n=22), miliary (n=5), tuberculoma (n=3) have been diagnosed. In all patients TBLB has been performed during bronchoscopy under anaesthesia. Results: The histological confirmation of the diagnosis has been gained by TBLB in 29 (43,3%). patients. Bacteriological diagnosis was confirmed in 71%. Tuberculosis was established at surgery or autopsy at three patients respectively. None of the patients developed TBLB related complications. Conclusion: TBLB is a relatively safe and fast method in the early diagnostics of pulmonary TB. However, besides an accurate bacteriologic examination of the sputum, bronchial washing and catheter biopsy, a surgically obtained sample could sometimes be additionally required to establish a definite TB diagnosis.
