Videothoracoscopy in early diagnostics of tuberculosis of pleura in the prisoners of St- Petersburg region

Abstract 15 patients underwent videothoracoscopy (VTS) for pleural effusion (PI) of unidentified origin. All 15 were males aged from 19 to 38 and had spent from 6 to 18 months in jail. None of them previously had had tuberculosis(TB) history and in all of them lung X-Ray, bronchoscopy failed to reveal TB.In all the patients effusion and sputum smears by Ziehl-Neelsen, same as effusion and sputum Levenstein-Iensen cultures - were negative. In 13 patients VTS was made under selective intubation, in 2 –under local anesthesia. In all of them fluid aspiration, division of adhesions and biopsy of demonstrative lesions of the costal pleura were made for further histologic examination and Levenstein-Iensen culture. In 14 patients (93.4%) TB was revealed during VTS and confirmed after histology in 13 of them (86.8%) and Levenstein-Iensen culture in 1(6.6%) with negative histology. In 1 patient (6.6%) TB was completely rejected after VTS and sample' examination. No major complications occur. VTS enabled us to reveal the high rate of TB–caused PI among prisoners.
