Applicability of exhaled CO measurement for calculation of CO transfer parameters in healthy volunteers

Abstract

Introduction. Earlier we established relation of measurable CO concentration in exhaled air to breathing maneuvers parameters, CO equilibrium value in alveoli (Co), inspired air (Ce), diffusion capacity (DL) and alveolar volume [Babarskov E.V. et al. ERJ 2001; 18, 33: 136S].
Aim. To determine Co, DL and Va with the help of established value of measurable CO concentration.
Methods. Equations for DL, Va and Co calculation were found by mathematical analysis of functional relationship with the help of 3 values of CO measurable concentration. Different Ce values at different parameters of breath holding time (T), breathing cycle time (Tc) and tidal volume (Vt) were taken into account.
Co concentration was measured by high selective laser spectrophotometry. Spirometric parameters were measured by ultrasound flowmeter in 10 healthy nonsmoking male volunteers (44.1±]11.2 years) at 5 different T-values (interval 0-35 s). All maneuvers were performed within 30 min. Three different values of measured concentrations were substituted into equations in ascending order (totally 10 combinations). Dl, Va and Co and their mean values in all combinations were calculated. For comparative analysis DL and Va were also measured by single breath (SB) method (MasterLab, Erich Jaeger GMBH).
Results. In comparison with SB parameter mean calculated Va deviated not more, than by 10%. Mean DL showed 1.5 fold increase compared with SB values. Co in different individuals was within 1.5-3.5 ppm with intraindividual week variability ±]25%. Co significantly correlated with Ce (changes between 0.3-1.0 ppm).
Conclusions.Method of DL and Va determination by exhaled CO measurement at different values of breath holding time could be developed and become helpful in CO transfer measurement.
