Effect in ventilatory measurement of new in line filter

Abstract We have tested a new disposable filter at the mouth on measurement of pulmonary function tests, by a controlled, random single blind study. The filter (Spirometry filter MADA, Italy), in characterized by hydrophobic fabric with tested good antimicrobial activity, low resistance (0,93 cm H2O/l/s at flow 12 l/s), little size (filtering surface: 24,62 cm2), low price. We have recorded the flow/volume loops with and without filter in 43 subjects (22 normal, 21 with obstructive impairment, mean age 51, 22 m, 21 f) by a Spirolab spirometer (MIR, Italy), equipped with a turbine. The recorded data showed a little mean differences between measurements with and without filter, significant for FVC and FEV1, no significant for PEF, FEF 75 and FEF 25-75. However the analysis of limits of agreements (Bland and Altman) showed that measurements of FVC and FEV1 were within 2 standard deviation (SD) and that PEF, FEF 75 and FEF 25-75 have homogenous distribution considering the intrinsic variability of parameters. Conclusions: the filter tested showed a tendency to a systematic little flow measurement reduction, clinically not remarkable, because these is within the normal variability of parameters. The filter, very cheap, may be used in normal practice in lung function measurements to reducing risk of cross infections.
