Abstract

Background
Experience with Ultraflex(c) expandable metallic stents in the management of patients with airway compromise secondary to a variety of endobronchial pathologies is reported.
Methods:
Between 1999 and 2002, 35 Ultraflex(c) expandable metallic stents were deployed in 30 patients (10 male, 20 female, median age 66 years) who presented with respiratory distress. Formal surgical intervention was inappropriate for each patient because of comorbid illness or extent of malignancy. Stents were deployed broncoscopically under direct vision.21 patients had intrinsic airway obstruction, five extrinsic airway obstruction, two had tracheal tear, one had malignant tracheobronchial fistula and one patient had combined intrinsic and extrinsic obstruction.22 patients had a tracheal stent alone and one of these patients had two tracheal stents. Five patients received bronchial stents. Three patients received both tracheal and bronchial stents. One of these patients had two tracheal stents.27 stents were covered, eight were uncovered.
Results:
All patients were successfully stented with restoration of airway patency and closure of tracheal defects. Satisfactory stent position was confirmed with follow up bronchoscopy in all. Late complications included halitosis (n=2), sputum retention (n=4) and formation of granulation tissue (n=1).
Conclusion
Ultraflex(c) expandable metallic stents should be considered in the management of airway compromise in selected patients.
