Abstract

The aim of the study: restoration of patency of the airway with the usage of stents in cases of infant tracheomalacia.
Material and methods: tracheomalacia is a process characterised by flaccidity of the supporting tracheal cartilage, widening of the posterior membranous wall and reduced anterior posterior airway calibre.
Two types of tracheomalacia occur in infant cases.
Type I is associated with congenital tracheal abnormalities in the structure of tracheal cartilage, most often reported with oesophagotracheal fistula. Type II is caused by coexisting vascular anomalies as there leading to pressure on the trachea. Cases of pathological changes in the structure of tracheal wall caused by prolonged intubation, infection or previously conducted operations, e.x: tracheostomy.
In years 2001-2002 5 infants in the age of from 5 to 18 month were hospitalised due to chronic inflammatory processes of tracheal infections and respiratory inefficiency caused by airway obstruction due to flaccidity of the tracheal, and had 8 stents used: vascular stents the Palmaz steel stent (Johnson &Johnson), expandable metalic stents the Palmaz stent S.M.A.R.T. (Johnson & Johnson) and polyflex stent (Rüsh).
Results: In all the cases the correct airflow, ventilation and tracheal functions were achieved. No negative influence on the development of the infants' trachea has been observed.
Conclusions: Use of stents to trachea or bronchi in cases of airway obstruction caused by flaccidity of wall enables to avoid a prolonged intubation or tracheostomy and provides with a physiological ventilation and the correct airway flow and an infant development without the risk due to aggressive treatment.
