Surgery for the eventration of the diaphragm

Abstract The eventration of the diaphragm is the rise of the diaphragm towards the thoracic cavity because of the decrease in tonicity due to congenital or acquired etiology. Of 74 patients with the diagnosis of the eventration of the diaphragm, 37 were considered to benefit from surgery after proper physiological evaluation. We have retrospectively analysied the symptoms, physical findings, type of operations and surgical results of these 37 patients operated at our institution between 1991 and 2001. We had 12 female and 25 male patients with the median age of 48 years (range from 16 to 73 years). The classical approach was a posterolateral thoracotomy through 7th intercostal space. Eight of the cases were on the right (22%) , 29 on the left (78%). The thinned-out diaphragmatic leaf was repaired with plication in 26 patients. A second method we used in 7 patients was Mayo reinforcement. In 4 patients we used grafts for the repair. We had one postoperative mortality (2.7%) and our morbidity rate was 16% (n=7). Mean hospital stay was 12 days. In conclusion, the eventration of the diaphragm must be surgically repaired when it causes symptoms and respiratory discomfort.
