Abstract BACKGROUND: Cystenil-leukotrienes (cys-LT) and isoprostanes are inflammatory metabolites derived from arachidonic acid. Exhaled 8-isoprostane is considered a reliable marker for lung oxidative stress. OBJECTIVES: The aim of this study was to evaluate the effect of a course of oral steroids on Cys-LT and 8-isoprostane levels in EBC of children with asthma exacerbation. METHODS:15 asthmatic children with asthma exacerbation (FEV1 70±]5% pred) performed EBC collection and exhaled nitric oxide (FENO) measurement (NIOX, Aerocrine), before and after a 5 day course of oral prednisone (1mg/kg/day). Cys-LT and 8-isoprostane concentrations were measured using an EIA (Cayman Che). EBC collection was also performed in 10 healthy children. RESULTS: Before prednisone therapy, both Cys-LT and 8-isoprostane levels were higher in asthmatics (Cys-LT 15.6±]3.2 pg/ml; 8-isoprostane 18.6±] 3.6 pg/ml) than in healthy children (cys-LT 4.9±]0.8 pg/ml, p< 0.01; 8-isoprostane 2.8±]0.26 pg/ml, p<0.01). After prednisone therapy, there was a significant decrease in both cys-LT (6.5±]1.2 pg/ml, p<0,01) and 8-isoprostane (10.6±]1.8 pg/ml, p<0,05) concentrations, but 8-isoprostane levels remained higher than in controls (p<0,001). FENO levels were not significantly correlated to cys-LT and 8-isoprostane concentrations. CONCLUSION: After a course of oral steroid therapy there is a reduction in cys-LT and 8-isoprostane levels in EBC of children with asthma exacerbation, even though 8-isoprostane levels remain higher than in controls. This finding suggests that corticosteroids may not be fully effective in reducing oxidative stress in children with asthma exacerbation.
