Abstract 8-Isoprostane, a marker of oxidative stress, is increased in exhaled breath condensate (EBC) of adults with asthma (Montuschi P. et al . Am. J. Respir. Crit. Care Med. 1999; 160:216-220). EBC is a non-invasive method of sampling secretions from the airways. The aim of this study was to quantify lung oxidative stress in children with asthma as reflected by exhaled 8-isoprostane levels. We also measured exhaled nitric oxide (NO), a marker of airway inflammation, and exhaled prostaglandin (PG) E2 , since impaired production of this eicosanoid has been proposed to contribute to the pathogenesis of asthma. Twelve healthy children (3/9 M/F, age 9±]0.5 yr, FEV1 96.3±]3.5% pred) and 30 children with mild to moderate persistent asthma (20/10 M/F, age 11±]0.6 yr, FEV1 89.9±]2.6% pred) treated with inhaled corticosteroids (average dose 300 mg/day) were studied. Exhaled 8-isoprostane and PGE2 were measured by specific radioimmunossays developed in our laboratory which were validated by high performance liquid chromatography. Exhaled NO was measured with the NIOX system (Aerocrine, Sweden). Exhaled 8-isoprostane was increased in children with asthma (47.2±]2.3 pg/ml; P<0.008; 95% confidence interval [CI], 42.4-52.0) compared to healthy children (34.2±]4.5 pg/ml; 95% CI, 24.3-44.0), whereas exhaled PGE2 concentrations were similar in the two groups (51.5±]4.0 pg/ml vs 46.8±]5.3 pg/ml, respectively). Children with asthma (37.8±]6.6 ppb, P<0.04; 95% CI, 24.3-51.4) had higher exhaled NO levels than healthy children (15.2±]1.7 ppb; 95% CI, 11.4-18.9). These data indicate that lung oxidative stress is increased in children with asthma treated with inhaled steroids.
