Abstract

Clinical observations indicate that respiratory disorders are common complication in alcohol and opiate abusing patients. The aim of this study was to evaluate the frequency of small bronchi obturation and COPD in abusers of alcohol and opiates. There were 251 persons under examination: 160 alcohol abusers (group I), 55 opiates abusers (group II) and 36 opiate dependent patients classified to the methadone maintenance treatment (group III). All dependent persons were treated in the Detoxication Unit of the Department of Clinical Toxicology CMUJ. Examinations were performed before treatment. All examined subjects were smokers. Ventilation efficiency was determined on the basis of the results from a flow-volume curve, spirometry and the measurements of the respiratory resistance ( Rrs) in a computer aided spirometer. COPD was diagnosed according to the guidelines of GOLD 2000. The frequency of small bronchi obturation in I and II groups was on the similar level and was observed in 1/3rd of examined persons, whereas in group III –22% of people had such obturation. Similarly in the group I and II the frequency of COPD was higher (18%) than in the group III (11%). The diagnosis of COPD was substantiated by Rrs measurements. The measurement of Rrs is very useful in clinical toxicology practise because it is not dependent on a patient's cooperation. It may be a link between pulmonological results and medical conscience of patients, as the best results were obtained in the methadone group.
