Correlation of lung functions with radiologic findings in coal miners: a report from a major coal mining area in Turkey

Abstract

Coal mine dust exposure is associated with accelerated loss of lung function. We assessed pulmonary function in a group of underground coal miners with a mean 15.6+/-3.3 years of underground working history (n=70, mean age of 41.2+/-2.8) who during previous mine surveys had shown radiologic abnormality (p1/1 or greater) and in whom working place had been officially changed. Major category of profusion was category I (82.8%), followed by category II (17.2%). Progressive massive fibrosis was not detected. Round shaped opacities were detected in 33 of 70 miners (47%) whereas the rest were mixed (53%). Among the opacities p type was more prevalent (97%) followed by q type (3%). We performed spirometry in this group of coal miners. Mean spirometric values were as follows: mean FVC(%pred) = 4.30+/-0.66 (97.88+/-14.68), mean FEV1(%pred)= 3.51+/-.60 (96.11+/-0.16) , mean FEV1/FVC (%pred) = 82.1+/- 6.24(99.22+/-8.13) and FEF25-75 (%pred)= 3.74+/-1.37(97.01+/-35.17). We performed Pearson rank correlation after categorizing radiologic findings both categorically (Category I and II) and by specific lesion (as p1/1, p1/2, p/s 1/1, p/s1/2, p2/2 , p/s 2/2, p/s 2/3, q1/2, q/t 1/2 ).Although worst spirometric values belonged to a miner with a q/t lesion, we found no significant correlation between the extent of radiologic lesions and functional impairment as measured by spirometric indices, and the underground working years (p>0.05).
