Costs of chronic obstructive pulmonary disease in Sweden

Abstract

Aim: The study examined the costs of chronic obstructive pulmonary disease (COPD) for the Swedish society, and analysed the costs among those with and without a doctor-diagnosed disease.
Material and methods: During 1999, telephone interviews were made with 212 randomly selected subjects fulfilled criteria for COPD, derived from the Obstructive Lung Disease in Northern Sweden (OLIN) population study. Of these, 72 subjects (34%) had a doctor-diagnosed disease. Classification as mild, moderate, or severe COPD was made according to the BTS guidelines. To reduce recall bias, the subjects were interviewed 4 times (in March, June, September, and December). The costs for the different groups were also transformed with the estimated prevalence in Sweden.
Results: Total costs were SEK 13,418 (EUR 1,448) per patient with COPD. The average total costs were about three times as high for those with a doctor-diagnosed COPD than for those without a doctor-diagnosed disease. With the individual costs measured in this study, the annual total cost to the Swedish society was estimated at SEK 9.1 billion (EUR 982 million). Although the average total costs for subjects without a doctor-diagnosed COPD were lower, the cost accounted for about 50% of the total costs in Sweden, because 66% of the patients did not have a doctor-diagnosed disease.
Conclusions: COPD is a costly disease for the society. We found that subjects without a doctor-diagnosed COPD accounted for approximately a half of the costs for the disease, indicating that register based studies of costs of COPD results in a substantial underestimate.
