Trends in hospitalization of patients with respiratory diseases in western Slovakia during 1992-2001

Abstract Background: Morbidity (incidence of disease), character of disease, need of treatment schedule are main medical factors (except of many others) defining the type of health care (hospitalization or ambulatory care). Methods: The evolution of number and type of respiratory diseases in patients hospitalized from our Institute¿s natural catchment area of about 1,5 million inhabitants were analyzed over the period of last ten years (1992-2001) by use of annual reports. Results: Decrease in total hospitalized patients with respiratory diseases, from 3765 in 1992 to 2513 patients in 2001 was recorded. In specific disease groups, drops in tuberculosis (from 528 cases in 1992 - ratio 14% from total, to 181 cases in 2001- 7,2%, index of all patients with TB in last five years 1997-2001 to first ones 1992-1996 -0,56), in COPD ( 803- 21,3%, 440-17,5%, 0,63, respectively), in bronchial asthma (560-14,9%, 257-10,2%, 0,59), while increase in bronchopneumoniae (441-11,7%,336-13,4%, 0,77), lung cancer (817-21,7%,1161-46,2%, 1,09) and in lung fibrosis and/or sarcoidosis (94-2,5%, 97- 3,9%, 0,83) were observed. We reason these changes by absolute drop in incidence of TB, increasing incidence of lung cancer and by progressive shift of management from in-patient¿s to out-patient¿s sector, especially in bronchial asthma and COPD. Pneumoniae remain mainly the matter of in-patient¿s sector. Conclusions: Decennial changes and trends in hospitalization from the same natural catchment area in west Slovakia reflect not only trends in morbidity, changes in ratio of in-patient¿s/ out-patient¿s sector, but by extrapolation show future evolution and needs for specific respiratory diseases in this part of central Europe.
