Abstract
Since 1974 till 2013 we observed around 3000 patients with proven sarcoidosis. Investigation included anamnesis, chest CT, US of abdomen, bronchoscopy with BAL and TBLB or VATS. All cases were confirmed by morphology. Non-typical variants of disease were observed in 66 cases (3,3%).Of them, further variants of clnical course were observed:sarcoidosis with symptoms of lymphoid proliferation (subfebrile fever, spleno- and hepatomegaly, peripheral lymphnodes impairment) – 12 cases.sarcoidosis with progressive pneumosclerosis and growing breathlessness (like in IPF cases) – 26 cases.Infiltrative and pneumonic forms of disease with conglomerates and atelectases in the lung (like in infiltrative TB) – 28 cases.Duration of observation over our patients reached 17 years in maximal. All cases were with recurrent form of disease. Overall mortality in studied cohort reached 13.6 % (9 cases).Conclusion: All atypical variants of clinical flow of sarcoidosis were prognostically non-favourable. All these cases need advanced investigation and prolonged and intensive treatment.
