GOLDs definition of COPD in a Norwegian community by self-reported clinical diagnosis of obstructive lung disease and respiratory symptoms

Abstract

No knowledge is available on the relationship between the GOLD (Global Initiative for Chronic Obstructive Lung Disease) spirometric definition of chronic airflow limitation and the presence of respiratory symptoms and clinical diagnosis of obstructive lung disease in a general adult population.
The aim of this study was to estimate the proportion of asymptomatic subjects with no diagnosis of obstructive lung disease in those classified as having COPD after the GOLD spirometric definition.
Methods : A random sample of two cohorts born 1925-27 and 1950-51 was drawn from the general population in Bergen, Norway. In 1998-99 a self-administered questionnaire was filled out on 7 respiratory symptoms and on 4 obstructive lung diseases (asthma, COPD, emphyzema and bronchitis) treated by a physician the last 12 months. Spirometry was performed before, and 15 minutes after inhalation of 0,4 salbutamol-powder. In accordance with the GOLD criteria, those with a postbronchodilator FEV1/FVC< 0,7 and FEV1< 80% norwegian prediction equations were classified as having moderate or severe COPD.
Results : Alltogether 3307 (94% of the participants) performed a technical acceptable bronchodilator test. The prevalence of chronic airflow limitation was 7%. 40% of the 228 with chronic airflow limitation reported a physicians diagnosed obstructive lung disease treated the last 12 months. 79% reported respiratory symptoms. 20% reported no respiratory symptoms or physicians diagnosis of obstructive lung disease.
Conclusion : In an adult Norwegian community 2 out of 10 subjects considered having chronic airflow limitation by the GOLD spirometry criteria reported no respiratory symptoms or physicians diagnosis of obstructive lung disease.
