Validation of the confronting COPD screening questionnaire in a respiratory clinic population

Abstract

The Confronting Chronic Obstructive Pulmonary Disease (COPD) Survey in North America and Europe aims to identify international differences in the clinical management of COPD and on the patients' perspectives on their disease in 2000. Cases were obtained from the general population by telephone interview screening and defined as persons with a physician diagnosis of emphysema, chronic bronchitis, COPD or symptoms of chronic bronchitis, older than 45 yr., and with at least a 10 pack-year history of cigarette smoking. Our aim was to validate the screening questionnaire used in this study in a sample of patients presenting at a respiratory clinic with COPD, asthma, or other respiratory diseases.
Interviews were conducted at an outpatient clinic of the London Chest Hospital (11/2001-1/2002). Of the one-hundred-and-thirty-six patients contacted, twenty-five refused or were not able to participate. Questionnaire data were compared with medical records and spirometry on 104 patients. The average age of the sample was 65.5 years (SD 10.3), the male to female ratio was 49%:51%. Overall, the questionnaire correctly identified 86.5% patients with a diagnosis of COPD and chance-corrected agreement (kappa=0.658, SE 0.098) indicates good agreement between the questionnaire and the medical records. The questionnaire had a high sensitivity (92.0%) and specificity (72.4%) relative to the diagnosis of COPD. The screening questionnaire used in The Confronting COPD Survey appears to be a valid tool to differentiate COPD versus asthma or other respiratory disease.
