Abstract

We have examined 1136 children both in the city of Irpen and White Church plus 1119 pupils from three schools in the city of Kiev. It established that mycobacterium tuberculosis infectiousness in children from zones with radioactive contamination is more considerable, than among children from other areas without pollutants- 37,8 % and 22,3 % accordingly, p < 0,05. Consideration of the results has allowed to reviling immunosuppressing conditions (ISC) in 58,5 % pupils with mycobacterium infection. The rate of a hyper-positive reaction to tuberculin skin test are calculated in about 2,9 % and 5,4 % in children with and without immune-deficit accordingly (p < 0,001). But the proof expressed type of reaction to tuberculin (diameter is greater than or equal to 15 mm) among TB infected children with problems in immune system is observed in 1,4 times frequently (22,9 % in group with ISC and 16,1 % without ISC, p < 0,001). The majority infected children with changes in the immune status had syndrome of weakness, even if not both TB diseases and infected with HIV. We detected clinical indications of secondary immune insufficiency (CISI). It is necessary in condition of financial deficit of maintenance of the medical programs, because immune test are valuable. Such way, the children in stage latent TB infection with CISI may be considered as a high-risk group for preventive therapy based on individual assessment of risk and benefits. The low efficiency of the chemoprophylaxis in these children, especially with having proof expressed and hypersensitivity type of reaction to tuberculin, leads to urgent search of new ways of its management.
