The eosinophil cationic protein/eosinophil count ratio as a marker of asthma in children

Abstract Serum eosinophil cationic protein (ECP) has been promoted as a direct marker of allergic inflammation and may reflect the degree of bronchial inflammation in patients with asthma, but its clinical value as a parameter for monitoring asthma has been widely discussed. We hypothesised, that the ratio of the serum ECP concentration to peripheral blood eosinophil count (ECP/Eo ratio), reflecting active eosinophils, could better correlate with bronchial hyperreactivity in asthmatic patients than each of these parameters alone. 165 children with mild or moderate asthma were included into the study. All patients were treated for their asthma according to established guidelines. At the time of the visit at the clinic, spirometry, inhalation challenge test with histamine (PC20H), blood eosinophil counts and serum ECP were measured. 140 patients completed the study. The ECP/Eo ratio correlated significantly with PC20H (p=0.002, r=-0.2577), but not with FEV1 (p=0.9, r=-0.01 ). Serum levels of ECP or peripheral blood eosinophil count separately, did not correlate either with FEV1 nor with PC20H. This data suggest that the ECP/Eo ratio might be a better and more useful marker than ECP or peripheral blood eosinophil count alone, in assesing asthma in children.
