Prediction of chronic renal failure in patients with chronic obstructive pulmonary disease

Abstract

Background: Chronic renal failure (CRF) rises in prevalence with age and is frequently associated with chronic diseases such as congestive heart failure and diabetes mellitus (DM). It is unknown to which extent COPD is associated with CRF and the relationship between CRF and COPD is undescribedRational: CRF may be associated with normal serum creatinine, a condition known as unrecognized or concealed CRF. It is diagnosed by glomerular filtration rate (GFR) < 60 mL/min/1.73 m2.Objectives: To investigate the prevalence and correlation of both concealed and overt CRF in patients with COPD.Methods: We measured the GFR for 527 subjects their ages ranged from 38- 78 years (327 COPD patients and 200 diabetic patient) using the Modification of Diet in Renal Disease (MDRD) Study Group equationMeasurements and Main Results: The main finding is the high prevalence of CRF (concealed and overt) among patients with COPD. As 66/327 of all COPD patients have CRF despite more or less accepted serum creatinine .The prevalence of normal and reduced GFR in COPD group were 80.1 % and 19.9%, while in COPD group with either DM or hypertention (HTN) it was 75.45 and 24.6%. The corresponding prevalence in DM group was 88% and 12%. Moreover, In COPD group 17.55% (46/262) had concealed renal failure (RF), while 1.52 % (4/262) had overt RF. In COPD with DM or HTN 18.46 % (12/65) had concealed RF, while 6.15% (4/65) had overt RF. While in DM group 10 % (20/200) had concealed RF, while 2% (4/200) had overt RF.Conclusions: our data support that CRF either concealed or overt are prevalent among COPD patients than among DM, this prevalence is further increased in COPD patients with other co morbidities.
