Oxygen desaturation events and pulmonary artery pressure in stable, moderate-severe COPD patients without respiratory failure at rest

Abstract Chronic alveolar hypoxia is the main cause of pulmonary hypertension (HPAP)in COPD patients. What minimum severity and duration of hypoxia is required to develop HPAP is not clear. AIM: to evaluate PAP, exercise related hypoxemia, 24hrs SpO2% monitoring in moderate-severe COPD patients in stable conditions with PaO2>=60mmHg at rest. Data of 18 patients are reported. METHOD:18 COPD pts (2F,16M,mean age 68,8±]2,6SE), FEV1%46,2(±]3,7), VR%121(±]13),PaO2at rest 73(±]8,6)mmHg,PaCO2 40,8(±]2)mmHg, DLCO% 55,4(±]5,7) were recruited. PAP was measured by Doppler Echocardiography. All patients performed 6min walking test (WT); if SpO2>=90% a cardiorespiratory exercise test (CRET)with cycle ergometer was performed. RESULTS (mean±]SE) PAPmmHg 33(.47), 8pts had SpO2 <90% during WT, 4 pts SpO2<90% during CRET (40W), 3 pts had no desaturations during CRET, 3 non desaturators did not perform CRET. 6 patients had 24hrs monitoring of SpO2% which revealed a mean of 1,5 hours/daily ±]1 of SpO2<88% Multiple correlation test did not show any significant correlation between PAP, respiratory function test, PaO2, PaCO2,exercise and 24hrs desaturations. CONCLUSION 24hrs SpO2% monitoring can add important information not only about nocturnal desaturations but also about desaturations during daily activities. To better understand the correlation between oxygen desaturation events and the development of pulmonary hypertension 24hrs SpO2% monitoring should be repeated many times yearly also during exacerbations.
