Abstract
Determination of predictive indices, which are significant in realization of transition to spontaneous breathing after long-term mechanical ventilation often is hard to be achieved in the early stages in the process of weaning from mechanical ventilation.
The study covers 45 patients who are ventilated mechanically and are made attempts for transition to spontaneous breathing. The patients are divided into two groups - group A (38 patients who perform successfully a 2-hour test for spontaneous breathing through Ayre's T-circuit) and group B (7 patients who perform unsuccessfully a 2-hour test for spontaneous breathing through Ayre's T-circuit). The monitored parameters in this period are respiratory rate/tidal volume ratio (f/Vt), occlusive pressure (Po. 1), inspiratory time/tidal time ratio (Ti/Ttot), pressure time index, pressure time product and work of breathing (WOBp.). The indices WOBp and f/Vt are most sensitive and have the biggest predictive value (p<0.001). They determine further strategy for transition to spontaneous breathing after long-term mechanical ventilation.
