Abstract

Objective: To determine the clinical profile and outcome of patients with Acute Respiratory Failure (ARF) from a developing Asian country. Materials and methods: Data on adult patients who were either admitted with or developed ARF during the hospital stay were evalauted over a four year period. Data was analysed using SPSS version 10. Results: Of a total of 270 patients, 80% were admitted with and 20% developed ARF during hospital stay. The mean age ±] SD was 60±] 14.2 years and 70% were males. The common underlying causes of ARF were acute exacerbation of COPD (56%), pneumonia (46%) and acute asthma (13%). Dyspnea (95%) was the commonest symptom and tachypnea (95%) and crackles (83%) the common physical signs. On blood gases pH was 7.31±] 0.11, PCO2 61.0±] 19.2 and PO2 60.3±]28. Mechanical ventilation was required in 35% and trachestomy in 3% patients. Drug therapy included antibiotics (96%), bronchodilators (92%) and steroids (76%). Nosocomial pneumonia developed in 14%, ARDS in 8% and cardiac arrythmias in 3% of patients. Mortality rate was 28%. Conclusion: This is the first study from Pakistan on ARF and shows that the disease spectrum leading to ARF and their outcomes are broadly similar to those reported in developed countries.
