Abstract Background : A major challenge in asthma management is to achieve high adherence to treatment in order to optimise asthma control. Poor adherence to asthma treatment is responsible for a significant part of negative outcomes.Aim : To identify factors that directly or indirectly influence patients' adherence behaviours and to structure the information in a comprehensive model.Methods : A model based on literature findings was developed and critically reviewed by 12 pulmonary specialists in the UK, Spain and France. Data from face-to-face interviews with a stratified sample of 45 patients were used to confirm the factors and to refine the model.Results : The main factors that influence adherence to asthma treatment were: patient–clinician relationship; patient's knowledge of the disease and its treatment; patient's beliefs and perception of the disease and its treatment; and patient's decision to take active part in the asthma management. The most important factor in promoting adherence was identified as the patient–clinician relationship because of the variability of the disease.Conclusion : The results show that adherence, from the patient's and clinician's perspective, can be defined as an optimal use of asthma medication to achieve treatment success in a variable disease.
