Abstract Large-cell carcinoma (LCC) of lungs is the most rare 4-th histologic type of lung carcinoma. 25 cases of this pathology were operated last 15 years. This number of patients composed 1,5 % among all cases lung carcinomas. Male to female ratio was 21:4 (84 % : 16 %), average age being 55,6 (38-82) years. 22 (86 %) patients were strong smokers. 9 (36 %) patients were asymptomatic. Most frequent presenting symptoms were cough (64 %), chest pain (24 %), weakness (24%), hemoptysis (24 %). Peripheral round tumor was found to 89,4 % cases. 9 (36 %) patients had marked leucocytosis and eosinophilia. Diagnosis was established by bronchoscopy in 3 (12 %), percutaneous biopsy in 4 (16 %) and by operation in 18 (72 %). All patients were operated. Extended process was discovered to 19 (76 %) cases. The most frequent performed operations were extended lobectomy (60 % cases) and pneumonectomy (28 %). Postoperative period was uneventful to 14 (54 %) patients. Level of intrahospital mortality was 12 % (3 cases). 4 (16 %) patients had appeared metastasis signs to early postoperative period. These results indicate LCC as asymptomatic and very aggressive tumor. To improve our treatment results, we have work on health education of people and early diagnosis of LCC.
