Overview of thoracoscopic findings between 1975 and 2000 in Hacettepe university hospital

Abstract Thoracoscopy is an invaluable diagnostic tool for the assessment of pleural pathologies. We evaluated the findings of thoracoscopies, that were conducted in Hacettepe University Hospital between 1975 and 2000. A total of 130 patients (86 men, 44 women) underwent thoracoscopy for the diagnosis of pleural effusion. Mean age and (SD) of the patients was 47.5 (13.1) years. Pathological diagnoses were malignant pleural mesothelioma (MPM) in 48, chronic fibrosing pleuritis (CFP) in 48, tuberculosis (TB) in 10, metastasis in 15 and not established in 9. Macroscopic features were nodular lesions in 25, plaques in 10, nodular lesions with plaques in 11, and tumour infiltration in 22. Plural thickening and adhesions were noted in 54, and 55 cases, respectively. Distribution of gender, pleural thickening, and adhesions were not different between MPM, CFP, and TB cases. Tuberculosis cases were younger than MPM and CFP (Mean ages were TB: 42.2, MPM: 47.8, and CFP: 49.4 years, respectively) cases, but the difference was not statistically significant. Presence of pleural nodules with plaques (MPM: 7, CFP: 3, TB: 0) and tumor infiltration (MPM: 14, CFP: 1, TB: 0) was significantly more common in MPM cases than CFP, and TB cases (p<0.05). Our analysis suggests that thoracoscopic finding of pleural nodules with plaques, and tumour infiltration may aid in the diagnosis of MPM. This possibility should be investigated in a prospective study.
