Abstract

Aim: To evaluate efficacy and reliability of pleural needle biopsy (PNB) in the diagnosis of pleural effusions of unknown etiology.
Methods: Retrospective analysis of 86 parietal PNB with Cope's needle performed at our department between 1998-2002. The patients were between 16-84 years of age (mean 45.4) and comprised 54 (62.8%) males and 32 (37.2%) females.
Results: In pathohistological examination of biopsy specimens were diagnosed: tuberculosis in 31 (36.3%) patients, malignancies in 30 (34.8%) patients, chronic pleuritis in 18 (20.9%) patients. 7 biopsies (8.1%) were not informative. The sensitivity of PNB was calculated 78.4% in tuberculosis and 58.6% in malignancies. Among patients with malignant infiltration of the pleura in the biopsy specimens, only in 40.0% neoplastic cells were demonstrated in cytologic examination of pleural fluid. Complication of PNB was encountered in only 1 patient (hemothorax).
Conclusions: PNB is easy to perform diagnostic procedure with low risk. It has high diagnostic accuracy in the diagnosis of pleural effusions of unknown etiology, especially for tuberculosis and malignancies.
