Pulmonary alveolar proteinosis – Gender differences in a 20 cases analysis in a reference institute in Romania

Abstract Introduction: Pulmonary alveolar proteinosis (PAP) consists of accumulation of surfactant in the alveolae; whole lung lavage (WLL) remains gold standard for treatment.Aims: To evaluate all cases with PAP admitted in a tertiar pneumology center in Romania during a six year period and to asses gender differences.Methods: All patients identified with PAP diagnosis were included in this retrospective study. All their admissions were recorded in a database containing demographic, clinical, paraclinical and evolution peculiarities.Results: We identified 20 patients (13 males), mean age at first admission 41.9±16.5 years, 4 smokers, 4 former smokers (all women non-smokers), 4 with exposure to respiratory noxes. Diagnostic was made by broncho-alveolar lavage in all cases. A full description was made with regards to clinical and paraclinical data. When evaluating the number of admission per patient, a mean of 2.23 admissions for men and 2.85 for women was revealed. WLL was performed in 14 patients (6 women, 8 men), a total of 30 lavages (2.5 WLL/woman and 1.87 WLL/man, p<0.001). According to these findings, women seemed to have more severe disease. When we analysed the differences between men and women we identified statistical signification in: mean duration of symptoms prior to the diagnosis (12.3 months in women, 5.3 months in men, p=0.001), haematocrit (p<0.001), number of erythrocytes (p=0.029) and forced vital capacity (p=0.006). No difference was noted for other functional test.Conclusions: PAP is not more severe in women, even if it requires more frequent admissions and WLL. This is, to our knowledge, the first study analyzing a big PAP cohort in Romania.
